how can i make a form pop-up on my wordpress site?

    Greetings, 

    I am having a heck-of-a-time getting my form ("Performers, avoid these 7 habits") to function as a pop-over or lightbox element (rather than embedded) on my website. Check out this site as an example of the ideal pop-up style that I would like, this type of element is known to increase subscribers up to 800%!  http://getresponse.com

    I'm having 2 problems:

    1. The form wants to originate in a link, rather than "popping up" automatically when a user visits the site.

    2. Several of the codes are causing major errors on my homepage (see image attached)

    Can you help me resolve these issues?

    And, please let me know if the type of element I'm looking for is possible with JotForm.

    I did check your web site but could not find the lightbox form embedded there. You seems to have a getresponse contact form. Please let me know on which link the lightbox should popup so that we can check if there is any conflict issue.
