How can I create a phone number field with a third box for extensions?

    The current phone number field has two boxes I need one with three so the third spot can be extension

    Hi,

    You can just add another Textbox field and position it on the same line as the 'Contact Phone Number' field in your jotform http://www.jotform.us/form/43105236756150

    Another suggestion is, how about if you just use the Input Mask property of the phone number field in your jotform. Like this..
