JotForm is a free online form builder which helps you create online forms without writing a single line of code. No sign-up required.
We believe that if one user has a question, there could be more users who may have the same question. This is why many of our support forum threads are public and available to be searched and viewed. If you’d like help immediately, feel free to search for a similar question, or submit your question or concern.
You have set your dropdown and input fields to be of size 15px. That is the reason why they appeared as being a form of small box.
I have clicked on the dropdown and change its width from 15px to 200px followed by a click on textbox and set the width to 250px.
You can see how the fields aligned here: http://form.jotformpro.com/form/43122613170946
If you want, you can clone it to your account: How to Clone an Existing Form from a URL
Or just enter the designer again and by clicking on the fields, change the width.
Do let us know how it goes for you and if you have any further questions.
Glad that we could help you.
Please go to this thread about your problem with radio button field: http://www.jotform.com/answers/456619. I moved it as a new thread because we can only deal with one question per thread.
