How can I put certain months and year at the “Date Time” field?

Sorry but it is not possible on the form builder. You can't set certain dates on the Date Time field on its properties. You may however use URL parameter to populate numbers on the Date Time field that will represent as the default date value.

Example:

http://form.jotformeu.com/form/43334215068349?gelisTarihiniz[day]=25&gelisTarihiniz[month]=12&gelisTarihiniz[year]=2014
