We had an error message almost every fields

We had an error message "A problem occurred with this web page so it was reloaded." almost every field.  Seems to happen more often in date field.  Every time it reloads, the data is lost.

Since I added page breaks and the Form Tabs widget I have seen this same error: I can't progress past the first page. 

After refreshing my screen the form updated, and I am having success submitting again.
