It will preview the image, but it is not uploading. I put in a feature request for a minimum file size on this widget last night, not sure if that has anything to do with the problem? This has our site crippled on one of our busiest days of the year, I need this fixed ASAP. I created a basic form for testing, you can click and click on upload and it does nothing. I tried it in firefox and chrome, same problem in both. screenshot below;

Apologies for the inconveniences that this might have caused you. Upon checking your account and cloning your form for testing purposes, I was able to replicate the problem.
You'll see in the bottom, where I have my console log that there's an error in the widget. It seems that it can't upload the file properly, although it can preview it.
I then created a separate form (http://form.jotformpro.com/form/43347321158957) and added the image upload preview widget again. And I was able get the same problem.
I will escalate this to our developers and technical team so that they can take a closer look. We will receive a notification from them when the problem has been fixed or if they have any updates.
For the meantime, I would like to suggest alternative widgets for file or image uploads.

Thank you, I am hoping this can be fixed soon! The other forms I use with this were working fine for some time, but I started getting emails from annoyed customers about 2:30 this afternoon which must be when it stopped working.

Don't mean to be a pain, but this is literally the biggest day of the year for my company. The workaround is okay, but it's not the same as the upload preview widget. If an increased sense of urgency can be placed on this I would appreciate it.

Again, my apologies for the trouble that you are having right now. I've already escalated this to our upper management and already informed one of my supervisors regarding this. Rest assure that they are already aware of this and are currently investigating where the problem might be.

I'm glad to hear that. I've made a sample form in my end and it seems that it is indeed now working fine.
Although, I see that the ticket for this bug is still opened, probably they are still making some test or making some final fixes.
Do let us know if you are still having problems with this.
