Reservation Form Conditional addition

Dear JotForm,

We are trying to make a reservation form. What we would like to do is have the group let us know how many will be coming #of participants and based on the # then have them fill out name, address and passport or identification # for each participant. Is there a way that we can do this?

I think you could accomplish the scenario by Hiding or Showing Multiple Fields at Once

So for example, you may add a dropdown or radio button with the number of participants they want to add, and based on the answer show multiple fileds.

However, we have some widgets to make life easier on this matter, they will allow you to add dynamically as many entries as you wish. Here they are :
