Width of the form fields extend the entire width of the form.

Yes, you can alter that to any preference you might have.

I would always suggest to open the preview in a new tab instead of having it in the preview window.

If you do that you will see the jotform like this: http://www.jotform.co/form/43497387197877

The reason why you see the name fields as wide as the jotform is because the width of the preview window is small and form is under the impression that it is being viewed on a mobile device and will resize to full width when the size of the screen (preview window) is less then 768px.

Please let us know if you would like to have the same width as shown when you take a look at the jotform above and if so we will help you to set it up like that.

The form alignment is perfect upon checking it using my android device. As mentioned, when your device screen is less than 768px, your form will be rendered in responsive mode.
