Check box option is selected by default.

7) The final tick box (Please tick if under 18) continually reverts back to it's default state with 3 default-labelled options, and often remains with a default 'ticked' status. I have asked about this in a previous ticket, but when you checked it had magically fixed itself. It's driving me nuts.
