I am not sure what happened, but for the past year, Ive listed several services for customers to choose from. When I logged into JotForm, those fields appear to have disappeared? Is there a way to look at past forms and try to recoup them back? Thanks! Paul

We would like to recommend you to check a form Revision History tool.
You will be able to restore the form using that tool if there is available a correct version of your form.
