Headers Not Showing Up

    Hi there... I have a form and when it's submitted it isn't showing the headers that I put in there. When you click on the option fields it just shows what you click but not the title of COACHING FOR or CONSULTING for. Can I have those show up when the email submission comes through??

    Hi Jason,

    Thank you for reaching out to Jotform Support. We can set a pre-filled hidden question to categorize your coaching and consulting options to achieve what you need. Let me show you how:

    • In Form Builder, click on Add Form Elements + button.
    • Choose Short Text and drag and drop it into your form.

    • Edit the Field Header as you wish (Coaching For… in this example) and click on the Advanced tab.
    • Under the Default Value section type in your desired pre-fill answer (Coaching For in this example).

    • Scroll down and toggle the Hide Field option to On position.

    Doing this will show the answers under the coaching for Short text answers. You can see the screencast below in which I set the same settings for Consulting For option:

    Ok great... I think I got it. But is there a way to not have it repeat on there?

    Also, the "Please Check All That Apply" shows up on the consulting but not the coaching line. Thank you!

    Hi Jason,

    In order to make your reports look the way you want, you can fully customize them by adding or removing content, and rows as shown in this screencast:

    The workaround provided above is fine, but if you want, you can also remove the dummy fields from your form and simply add a row for each heading to the email template as shown on the screencast above. You can also add the missing “Please Check All That Apply” string to the email in the same way.
