I have had 2 registrations which have come through via email which are not capturing in the report and the email is not displaying the date and time which are mandatory fields?

Actually, everything is working fine. What you receive through email is coming from this form: http://www.jotform.co/form/50059310861853. This form doesn't have the Date and Time field.
Your screenshot that shows the form submissions page is for this form: http://www.jotform.co/form/50057388077865.
You have the same notification email format for this two form so they look likes the same, but it's not. I would advise re-creating your form notification email so you don't get confused.
a. Delete the existing email notification/autoresponder: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder.
b. Re-create the notifier: http://www.jotform.com/help/25-Setting-Up-Email-Notifications.
c. Re-create the autoresponder: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder.
Thanks for your response however why are we receiving 2 email confirmations – I will forward them both?
