Having Difficulty Accessing Some Forms?

Having difficulty accessing some forms. The embed link is:http://form.jotform.com/form/.............   and when following it times out and gives me "page not found" 

The strange part is, if I alter the link from jotform.com to "jotform.ca  it works just fine.  The extension should not have any issue.

Anyone have a solutions (other than altering the link extension)?

Upon checking your account and the forms you have, I was able to view them properly on different web browsers. It might have been a temporary hiccup on the server or something related to the a network issue. I've cleared your form's cache just to be sure.
