You send an email and I should be able to reply to your email. This is not a very good way to reply. What you sent in one link was my form. That form looks good but when someone fills it out it comes to my email not correct.
The other link has some impossible instructions to see or follow. As I said in my email reply is there any way for a phone call or can you send me the link to paste into my website that will have the whole filled out form come to me?

AGREEMENT TO MEDIATE AND ARBITRATE DISPUTES – Doctors Vitamins LLC and its IBOs mutually agree to resolve all claims and disputes arising out of or relating to an Independent Business, the IBO Contract as defined in Rule 1.1 of the Doctors Vitamins Rules of Conduct ("Rule" or "Rules"), as well as disputes involving Business Support Materials (“BSM”), as defined in Rule 7, under the Dispute Resolution Procedures described in the Rules, specifically Rule 11. The Rules shall be part of this IBO Registration Agreement (“Agreement”) and are incorporated by reference. A copy of the Rules is available to review at www.DoctorsVitamins.Org
I agree to submit any dispute I may have with another IBO, a former IBO, Doctor Vitamins LLC, as defined in Rule 7, to conciliation under Rule 11.4. I further agree that if any claim or dispute cannot be resolved by good faith efforts in conciliation under Rule 11.4, I will submit any remaining claim or dispute arising out of or relating to my Independent Business, the IBO Contract (including any claim against another IBO, a former IBO, or any such IBO’s officers, directors, agents, or employees; or against Doctor Vitamins LLC and any parent, subsidiary, affiliate, predecessor or successor thereof, or any of their officers, directors, agents, or employees) to binding arbitration in accordance with Rule 11.5. The arbitration award shall be final and binding and judgment may be entered upon it by any court of competent jurisdiction. Demand for arbitration shall be made within 2 years after the claim arose, but in no event after the date when the initiation of legal proceedings would have been barred by the applicable statute of limitations, subject to the tolling provision in Rule 11.5.5. it may be amended from time to time, shall govern the interpretation and enforcement of the arbitration rules and arbitration proceedings in Florida. The agreement to conciliate and arbitrate under Rule 11 is reciprocal and binds Doctors Vitamins, IBOs and Approved Providers.
The Arbitrator, and not any federal, state or local court or agency shall have exclusive authority to resolve any dispute relating to the interpretation, applicability, enforceability or formation of this
Agreement, including, but not limited to, any claim that all or any part of this Agreement is void or voidable.
BUSINESS SUPPORT MATERIALS – Some IBOs and Doctor Vitamins LLC accredited third-party providers (“Approved Providers”) offer optional BSM to IBOs (See Rule 7 for more information). I UNDERSTAND THAT I DO NOT HAVE TO BUY BSM TO BECOME AN IBO AND THAT BSM MAY BE HELPFUL BUT ARE NOT NECESSARY TO BE A SUCCESSFUL IBO. BSM ARE OFFERED SEPARATELY FROM THE Doctors Vitamins BUSINESS BY IBOS AND APPROVED PROVIDERS THAT ARE NOT OWNED OR OPERATED BY Doctors Vitamins. I further understand that Doctors Vitamins does not earn any revenue from my purchase of BSM from IBOs or Approved Providers; some IBOs and Approved Providers earn income (which is not paid by Doctors Vitamins) from the sale of BSM to IBOs; IBOs and Approved Providers are required to meet Doctors Vitamins’ Quality Assurance Standards for BSM (See Rule 7); I may have rights to a refund from the seller under certain circumstances; in deciding whether or not to purchase BSM, I will use my own good judgment and ensure that expenditures are consistent with my goals and with the profitability of my business; and, if I choose to purchase BSM, I will do so only after I have registered as an IBO, and after I have assessed my own needs for BSM. In addition, Doctors Vitamins offers optional free training for IBOs in marketing and merchandising.
REGISTRANT(S) - I certify that all of the information above is complete and correct, including my sponsoring IBO. I have read and agree to adhere to the terms of this Agreement, including the Doctors Vitamins Terms and Conditions printed on the reverse side (Page 2 of 2). Our apologies for the inconveniences caused.
Am I right that some information or answers are missing on the notification when it lands on your email? If so, recreating your email should resolve the problem. Unfortunately, once you made any changes on the notification it will automatically stop from updating. You have to recreate your notification in order to get the latest changes (e.g. new fields) in your form.
To do that, simply delete the existing notification and then create a new one.

Are you now happy that I am responding to your tread that all I get is an email that if I reply to it no one reads? Emails are much much better than treads. I suggest you use email and have better customer services.
As I responded to the last 2 treads. This does notwork. Please do it so I can receive the whole completed form. As you see above it looks messed up. I tried to follow the instruction but it will not work for me. Please do what is needed to have the whole completed form come to me and then please send me the code to enter on my site.

The instruction I received did not work. I was told someone on your team fixed it but I still do not get the whole form. Please fix it since I can’t. Please also stop ignoring my request to contact a supervisor or manager or owner of Jot Form to get this fixed.

Did you follow the instruction by our colleague EltonCris on how to Add the new Notfication Email on the form?
Click on the Add New Email button to add new Notifier in the form.

Ok I want to receive the form with all the filled in answers as it is seen by the person filling it out. I want it to come to my email complete with the answer. Look at the form and fill it out and make is so when you submit it I see what you saw the whole form from top to bottom.

Provide your own email address in the E-mail field so that you can received the Autoresponder E-mail.
If done correctly, the email message you will received will be similar to this
it includes the TEXT and Headers in the email message, including the questions and answers as well.
