I am unable to view my form when I log in, therefore I am unable to edit the form or view submissions. Hopefully it wasn't deleted! The web page is still available.

I do apologize for the issue that you are having with your form. After reviewing your account, I am seeing that your form is present.
Please ensure that you are currently signed into the correct account. If you are currently signed in, please ensure that your "My Forms" folder is selected as you may be viewing an empty folder/shared folder.
If you are still having an issue, please sign out of your account, clear your browser cache and sign in once again.

Thank you for your response! I have selected both the "My Forms" folder as well as the individual folders but nothing appears to the right. I have also tried clearing my browser cache which did not help either. Furthermore, I have signed in and out of the account but still, my form does not appear.
Not sure what else to do ... Please advise.

Thank you so much for your great tech/customer support! It looks like the form was accidentally filed away under "Archive"! Whoops! Problem resolved.
