Firefox problems with embedded form

Hello,

I cant seem to make the hide feature stick when using some browsers sometimes.  

For example:

for me at the minute on firefox does not work (but does work sometimes for some people on Chrome and IE (Sometimes!) ) 

you should be able to click individual, charity or church and it would then give you a different list for each... but sometimes it shows you all of the variables... 

any thoughts?

After inspecting that page, I found some jquery references that might be in conflict with the form's code.

I would recommend you to replace the current code of the form with iFrame Code
