There are actually 2 reasons because of which your account is disabled. Both are related to jotform titled "Clone of CV Submission".
The second issue is that the form is set as English Form, yet non English language is on it.
To have your account enabled, you either must remove the jotform completely or correct the issues above by renaming / removing password fields.
The second issue can be easily fixed by setting up your jotform for the right language.
