Getting direct photo link from Flickr

my flickr photo is not showing on my form

The Flickr's sharing option does not provide direct image links. In order to get the direct links, you can go to Download icon -> View all sizes.

Then, select the size you need, right click on the image and copy the image address.

Use the image address on the form payment tool.

You might also want to add a Product Lightbox widget to the form, so you form users will be able to click on the images to enlarge them.
