i have added Matt@tulsadrillers.com to the form for automatic confirmation emails but its not working.
he should receive emails when someone purchases a kids club membership online using this form.

As far as I can tell, your notification is properly set up:
I noticed you capitalized your address in your question but not in your notification. Our notifications can be fairly picky with addresses, occasionally, capitalization issues such as this do matter.
If that is not the case, let us know and we will be happy to see what else we can do. As far as I can tell though, your notification is properly set up.
