Looking at your email history log, it seems that you are testing email notification and the emails are being delivered to your account registered email address. Please be noted that clicking on Test email button, the notification shall be sent to the JotForm account registered email address only. If you have chosen to send the notification to a different email address, the form should be actually submitted as a normal user does it. Please try submitting your JotForm normally to test if the email notification sent to the address set in the recipient email address field.
