Could you please give us the link to the exact page where the jotform is found on?

I believe that there is a conflict and when I took a look at your other threads I found this page:

Where there is a conflict with the form and the code on your website.

To fix this, please use the iframe embed code instead: How to get the jotform iFrame Code

Once you do that issue will be resolved.

Do note that since a conflict is being shown on that page, you should use iframe embed option on all of your pages as they too would not be loaded properly due to the JS conflict.
