We would like to shorten our Child-Preregistration form so that parents can register one child, and have the option to add more children. I saw that it is possible to add a text box, but with each child come several other questions (age, grade, allergies, etc...)
Is it possible to add all of these fields per child added? What widget should I use for that?

Hello KristenBurke,
Yes it is possible to achieve your requirement.
I would suggest you to take a look at our "Configurable List" widget which should allow your user to duplicate the fields.
Another option is to take advantage of our "Conditions" feature to show/hide form fields based on the number of child user wants to add. You may also like to take a look at the following guides on how to show hide multiple fields:

http://www.jotform.com/help/297-Show-or-Hide-multiple-fields-at-once-using-form-collapse-tool
Please take a look at the following cloned form where I have added the required conditions and see if this is what you wanted to achieve: http://form.jotformpro.com/form/50774650890967?
Feel free to clone this form for a closer look. The following guide should help you in form cloning: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL
Hope this helps.
