Email notifications are sending the custom header only without any form fields data

Hi we have a problem, we are trying to use your site (which seems very good) to be our booking form for our community centre. It seems to be working well BUT when a form is complete and emailed to us we are only getting the heading we have put not the details of the form?

Could you please tell us if you have made any changes to your jotform's email notifications since when I am looking at the email log I can see that the emails are properly sent to you from our side just as is the test email:

I would just like to check also, if you get the correct email when you send the test email, but not when you submit the form itself - with the test data manually being added?
