Around March 20th I stopped receiving email notifications of any form submissions. I was made aware of this by individuals calling me wanting to know if I had received their form submission. I can log in and see the submission, but I am not receiving the email notification making me aware there is even a submission out there.Page URL:

Upon checking your form, it seems that you are missing the "Sender Email" in your form notifications, that may cause problems.
I have checked your email z**********dy@wint**w**ds.org, our mail log does not show error or failed email attempts. It is also not in our bounce list. It is highly possible that the email service provider for your email address might have started blocking our JotMail IP addresses and domain names in their end. Please contact them and ask to whitelist us, please refer to this guide: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses. After that, make a test submission by filling out the form and click the submit button.
