No email Notification being sent

I was getting email notifications when people used this form, but they stopped.

I checked your jotform http://www.jotformpro.com/form/22823895966976

and I noted that it does NOT have a Notification Email setup on it.

This was the reason there was no submission notification email being sent when form is submitted.

You can add a new Notifier using the Add New E-mail button.
