Can I map my forms responses to my sql database ?

The script would be uploaded and run from your own server.  You post the data to your thank you page:

http://www.jotform.com/help/51-How-to-Post-Submission-Data-to-Thank-You-Page

Then it is passed along to your database via your script.  Once your script is created and uploaded to your server, as per the guide:

Basically your thank you page will be your PHP script.
