Getting Stuck with 'Please Wait' on Simple Form

Hi!  I am having a problem with one user that is unable to submit the form here: https://secure.jotform.us/form/50964131925153 It works fine for me and other users, just one person is reporting this problem thus far.  Are there are suggestions I can have them try to submit the form?

I'm not embedding the form in a website, I'm using the direct link to the form via email to the recipients.

I just tried your form, and it works perfectly fine, that must be happening to him due to browsers cache issues, I would definitely recommend clearing browsers cache.

Asking to try in different browsers, we recommend using Chrome and Firefox. If he can not still submit the form, let us know in what browsers version the problem occurs for us to try to reproduce the issue.
