2 column radio button choice, can't make it work!

I'm trying to create something that looks like this:

*  Bar Tender       $9.00

    Server

*  Cook                $10.00

I have about 10 positions I need to list but allow the person completing the form to put the wage in next to the positions they choose.

Any help with be greatly appreciated.

Do you want to have single text box beside the radio button to enter the wage? If so, the configurable widget could be the right choice. You may also consider adding the options to a drop down box.
