We have been having a lot of issues recently withe google spreadsheet integration. Randomly the google doc will have missing fields, and sometimes gets completely reverted to an old format. Please explain how to keep this from happening.

May I ask you to try to create a new integration in your form?
I mean, remove the current integration and create a new one.
I'm asking you to do this because we had some glitches with the Google Spreadsheet integration over the last few weeks and our developers have released several patches to solve this situation. Creating a fresh integration will make sure these fixes are applied to your form and this should solve the problems you're having.
