When a form is submitted, I open it in the Inbox…the results do not include the headings, dividers and text blocks. How do I get these to show up? It's for Huron Dental new patients and they need to print the form and have the patient sign in office.

If you want the headers, divider and text blocks to show up in the email you will have to add it manually in the notification email.

You can also include a link of a PDf form of the data with the headers enabled

Goto to submission under the form whose data you want to include headers.

Check the Show Header and Texts option, this data will now be included automatically in submission PDFs

You can include a link of this PDF in your notification email, open your notification email and include the PDF link in the content Body.
