The form we have provided asks for the main applicants name and there is a spot for the applicant to enter in the name but in the last few days the name has not shown up on the form when we pull it up on Jotform. How to we fix that?

Can you please explain in more details from where applicants name is missing? Is it missing from the form or from submissions? Also can you please provide us ID of the form so that we can inspect it?
If you have deleted field from your form you can always add a new one inside of your form Builder.
If you are missing some fields values from your Email Notification, you can check your Notification to see if maybe you did not connect your fields with appropriate values of those fields.
