Recreate Matrix table inside of my form

    I have converted my forms from FormsCentral, but when I export the survey data to Excel, a heading gets embedded in the questions. This does not work for me. I have tried to re-create my form using JotForm tools, but cannot figure out how to do it. The creation tools are not very intuitive and are very inflexible. I need a 6-answer radio button matrix type form that is 7 pages in length. It does not appear I can create this.

    I have also tried modifying the form that was converted from FromsCentral but it appears this form cannot be modified.

    There are 5 other churches in my area that use FormsCentral and we all have the same form.

    Situation that you are mentioning is normal, because every row in you matrix is representing one option that can be selected and header will be always shown inside of your reports. 

    Possible workaround is to remove your header and leave it blank and add that removed header text inside of Hover Text.

    That way you would not have header text inside of your Excel report.

    Matrix table is located inside of Survey Tools inside of your Form builder. And you can change matrix table settings when you open Properties.
