Blank Submission

    Hi,

    I've received a couple of blank submissions for one of my forms, so I am concerned that something is wrong with the form, or something is unclear in the submission process. What can I do to fix this? The name of the form is Nomination Form 2015-2016 (link below).

    After clicking Next button, the next page is displayed, the form runs smooth until the last page.
