Form is gone from account

    Hi, We're using your form on our client's squarespace site. We recently changed the code block settings and it messed up the form, so I thought I would redo the iframe code, but it seems that the form is blank on the Jotform account. Please help

    Are referring to this form 51156243586962? I checked the form's revision history and noticed that the form's fields were removed/deleted recently. That is the reason why the form is blank. See screenshot below.
