Can you please give us more details as of how the form doesnt work, Do you have a blank screen or an error message? you can maybe include a screen shot of the issue.

No it is not a form i created, it is you form to allow me to sign up with jot form

It looks like you have already created an account with the username "jstevenscott" associated with your current email address. It will not allow you to create another with the same email account. You can send a password reset email for the account from the login screen if you need to do so.
