Date picker is not working on my form

We are sorry for the inconvenience. I have checked your forms, and I only see Date Pickers on two of them - Availability and Reservation request forms. I have tried using both of them, and the Date Picker seems to function fine:

If you have embedded this form on a website, can you please post a link to that website, so that we can examine what seems to be causing the issue?

Please describe in what way is the Date Picker not working for you - is it that the popup calendar is not showing, or is it something else?
