The person who set up the account no longer works for the organization. I'd like to change the username. Please advise how to do that.

We can change usernames for our user's account provided that the request will be coming from that same account that is requested to have a username change. Kindly create a request using the account that you would like to have a username change. Or if this is already the account, kindly give us the new username so we may proceed in changing the username for you.
Also, you need to confirm first that you are NOT using http://www.jotform.com/help/261-How-to-create-a-Custom-URL-of-a-form
Because changing the username will break the custom URL name of your deployed form.
