One of my forms is not working

hello, my name is Dave, and i am running a MC server, i am trying to make a form, but my second form doesn't seem to be accessable, could you please help me?

I checked your account and it seems like it was automatically suspended by our Phishing Detector which scans/detects forms that are made for phishing. I manually checked your forms and it looks like there is nothing suspicious about them. However, I noticed that you have one form - named Title Me - with only one field. To avoid such problems in the future, please avoid creating forms with only one fields or has default Title Me title.
