I need a form I can add to my web page lagowranchllc.com a birders' paradise where guest that have toured on our ranch can fill out comments .

I need the form to enable people who have toured our ranch to not only write comments, but add photos they have taken on the ranch with descriptions under their photos. I haven't added one of your forms yet, as I don't know which one you have that would do this for me.  Could you e-mail me your suggestion and on how to add it to my jimdo web site so it would work there?

If I understand your requirement correctly, you want to allow your users to be able to upload an image, write description of image and also the comments as well. Is that correct?

It seems you also want to display the user upload & comments in your web page. Is that correct?
