Is there any advice available on this matter?
On a Windows workstation running IE v11 an "Incomplete Values" message is being returned upon clicking "SUBMIT" button indicating four DATE fields are incomplete, despite being complete. On my Mac workstation, tested in Chrome and Safari, the form submits without any problem.

I have tested your form in IE 11 and I was able to submit your form. I have also tested my cloned form on IE 8 and Chrome and I was able to submit the form without errors.
I would suggest you to clear your Internet Explorer cache.
Can you test it on some other computer and see if the problem persist?
