I want all fields in my form to populate in my notification email. However, there are only a select number of fields showing up in the "form fields" list. How do i get the WHOLE form to appear in my email?

When you create a notification, all fields are included by default. If some of the fields are missing, simply recreate your email notification or autoresponder. Delete the existing notification, then create a new one.
Actually, when I check your form it has all the fields shown in your screenshot under form fields. The address is a group fields so it is listed as one.
