    I have created a new form and and we are in 'testing' phase. I've sent the form to other committee members and they are unable to open.

    Above is the form link. I did notice there are several CSS errors (but I don't want to change something I don't understand).  The form seems to work for me but for those accessing the link it is requiring them to log in.  

    I have the form enabled and it is not a 'secured' form.  Thank you in advance for your assistance.

    Your form is working fine. I could not find any errors. Tested on different browsers and location.

    It is strange that they are prompted with a login. When they open the form URL, they should see the form not any other else (even if its https or not). 

    If possible, can you ask them to take a screenshot on how the form appears on their end or the login they are seeing?

    Thank  you for your assistance.  They were able to open the form.  It
    seems the original link had a www. preceding the link which was
    preventing them from opening the form.
