How do I get my cover photo to show up on my pdf form

I clicked include in form but it is still not showing up

The form cover photo only applies when the form is used on the web, it is excluded in the PDF since PDFs do not support some aspect of the designer such as CSS based images hence the reason it is not showing up.

You can use the image element to add images onto your form this will allow them to appear in the PDF generated
