Could kindly try to re-embed you form, I have cleared you account cache.

Do the above and then revert back with the results.

Hi, I have checked you form, it seems the issue is with the conditions, the visible fields are set to be hidden in your conditions that's why upon preview only two are shown but in design mode all elements are shown including the hidden check-boxes.

Kindly revise your conditions to see if this resolves the problem

Hi, I will check this but take in consideration that the differences (modifications made) are referred to the Check boxes Questions (the text) which is different between OLD and NEW versions and should have nothing to do with Conditions unless a bug is present. Yesterday the for was running perfectly…

Maybe all this could be an issue due to the fact that I used the Adobe Muse Widget to create the form instead of just placing an HTML code in the body of the page.

I have open another ticket for a problem with this same form about Paypal here: http://www.jotform.com/answers/608722-How-solve-PayPal-error-when-users-click-Continue-Shopping-inside-PayPal-Site#3

However I still do not understand why the revision history declares correctly the CURRENT VERSION with the new texts, but when I go to the Edit Mode of the form I get the OLD one with the wrong text.
I suppose I have to rewrite the whole thing from the beginning.

Kindly try to clear your browser cache and login again.

Here is the form I see when I'm revising the issue

Kindly confirm if it is correct according to the screenshots above.

You don't have to recreate the form, kindly try to clone the form and see if the cloned version has this issue too

Well, Sammy, finally now I can see both in Edit Mode and History the right form with rigth data in it. You probably are right about the cache issue.

Sorry if I contributed to loose some of your time for a matter like this and thank you very much for your precious support.

    Ciao

    Enrico
