I've had at least one client who has filled out my new Jot Form that I never received. I'm concerned there might be more because I've directed other clients on the phone to that form. My tests are always successful however.

I have tried filling out the form on my PC and it went through okay, On the phone the starting time input is a bit hidden making it hard to select in portrait mode.
I think this could be the cause of the issue, kindly try enabling responsive mode in your form then test it out on mobile
PS. I did the testing on google chrome on android.
