1 form with several unique email submissions

Hi, I have a client that would like to create a single form that can be distributed to several of their client's websites. When the form is filled out the submission would go to the specific client's email address.

Is something like this possible? Or would I have to Clone the form each time a different email address is required for submissions to be sent to?

The reason they would like 1 form is because they anticipate changes being made to the form and it would be great if the changes only needed to be made once instead of on all of the Cloned forms as well.
