Active content doesn't seem to be working

Date widget not working, print button not working, next button not working, and I'm not getting copies of email. What is happening today? HELP!

I removed the next and print buttons and can enter the date manually but this stuff is supposed to work. What is hgappening?

I just sent a test using the direct link of the form, can you please tell us if the problem continues?
