I did check your account and did not find any issue with that. You do not have any form in your account. There are two forms shared with you by username "NHMU" and you should be able to see them after clicking on the "NHMU" under "Shared with you" folder. Please check the screenshot below:
If you are not able to see the shared forms, I would suggest you to please clear your browser cache and try again. The following guide should help you how to clear browser cache: http://www.wikihow.com/Clear-Your-Browser's-Cache
