Why is my form no longer working on my website?

We had received fully complete submissions previously.

As of yesterday, some people completed the form but could not submit it.

Other forms were submitted incompletely - with required fields missing.

Please help.

I checked your form and the form itself while not embedded appears to be working properly.  I was able to submit my clone of your form without issue.

Try replacing your embed code with the iFrame code:

http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

This should allow your form to function embedded the same way as it does while not embedded.

If this still does not allow your users to properly fill out the form, let us know and we will be happy to have another look.
