On one form I have loaded an image. On another form I cannot load a different image. Both images are in JPEG format and are the same size.

Looks like the image is not available: https://s3.amazonaws.com/jufs/EdLemond/form_files/SECNAV#1c.jpg. Probably because of an error during the upload process.
Please try it again, but before doing that, change the file name and remove any special character to it. Let us know how it goes.
